WRONG MEDICATION

For the leading Research on issues of Medical Negligence and Malpractice it is useful to look to the USA and what is happening there. Federally Funded research does much to identify and analyse issues surrounding Patient Safety. A Study in the Journal of General Internal Medicine has reported that an unsettling percentage of Death Certificates sampled in the Study has recorded death as having occurred in a hospital setting due to medication errors.

There are four points of interest in the medication use process:

  • When the medication is ordered (written, electronic or verbal orders)
  • When the medication order is transcribed and verified
  • When the medication is dispensed
  • When the medication is administered

The errors identified in the Study were:

  • Drug overdose
  • Wrong drug prescription
  • Consumption of wrong medication
  • Accidents involving medication during surgery or medical procedures

The Study found older adults to be more at risk with the complicating factor that such patients may be on multiple medications of varying strengths with the potential for an administered drug to either counteract the effects of another medication or to cause a severe reaction when combined with that other medication.

The Lesson to be learned from this Study is not to be afraid to QUESTION !
